Tichoepithelioma — the benign tumor developing from hair follicles and containing ferruterous cysts.

Arises mainly at young and youthful age.

Solitary tichoepithelioma (not inborn).

Multiple (inborn) tichoepithelioma.

Desmoplastic tichoepithelioma.

Etiology and pathogeny

The tichoepithelioma is sometimes observed in combination with tsilindromy since between them there is a close gisto-genetic linkage. The family cases connected probably with an autosomal and dominant mode of inheritance are frequent.

Clinical signs and symptoms

The solitary tumor has a fibropapilloma appearance.
Multiple elements is located mainly on a face and a trunk. Initially small small knots which diameter seldom exceeds 0,5 cm arise on skin of nasolabial folds. Gradually the quantity them increases, they extend paranazalno, taking skin of a forehead, pilar part of the head, a neck, interscapular area, auricles, sometimes leading to occlusion of outside acoustical passes. In many cases it is expressed dominant kraniotsefalny "эпителиоматоз".

The diagnosis and the recommended clinical trials

The diagnosis is made on the basis of results of clinical inspection and a biopsy.

At pathomorphologic research usually on a limited site under not changed epidermis complexes, various by the size and a configuration, which range can include the primitive "kidneys" similar to an embryonal stage of a hair follicle, and formation of very fancy configuration to existence of the structures reminding partially created follicle are visible. Cells in complexes of obviously ploskoepitelialny character, and on the periphery are located palisadoobrazno. Forming of a set of the keratinized structures like "horn pearls" is noted. Often find melanin granules in a keratinization zone.
In a tichoepithelioma there can be tubular (adenoid) structures. In certain cases in addition to cellular complexes a large number of ploskoepitelialny cysts is noted. Sometimes contents of cysts are exposed to calcification.

Differential diagnosis

Bazalioma with a piloidny differentiation, a tuberous sclerosis of Pringl — Burnevil. Reaction to an alkaline phosphatase indicates existence in a tichoepithelioma of rudimentary hair complexes. Besides, at a tichoepithelioma the stromal cellular infiltration consists of fibroblasts while at a bazalioma with a piloidny differentiation the stromal infiltration on the periphery of tumoral complexes consists of lymphocytes.

General principles of treatment

Electroexcision, laser evaporation, dermabrasion are shown.
