Neurilemoma

Neurilemoma — a benign tumor from neurolemmocytes of cranial or spinal peripheral nerves.

Epidemiology

Arises at middle-aged persons, it is equally frequent at men and women.

Classification

Allocate a neurilemoma:
• like Verokai;
• fascicular type.

Etiology and pathogeny

It can be combined with a neurofibroma at an illness of Recklinghausen or other, mainly malignant tumors of skin, mammary gland, gastrointestinal tract.

Clinical signs and symptoms

It is localized in hypodermic fabric and head skin, a trunk and extremities on the course of nervous trunks. The tumor is usually single, a rounded or oval shape, is more often than a dense consistence, pinkish-yellow color, with a diameter no more than 5 cm. Can be painful, sometimes with irradiation of pains on the course of the corresponding nerve.

The diagnosis and the recommended clinical trials

The clinical diagnosis is made on set of signs. At the small size of a tumor the ekstsizionny biopsy is shown, at the big size — a biopsy.
The tumor encapsulated on a histologic structure is subdivided into 2 types: Antoni's type of A and Antoni's type of B. At the first type the tumor consists of the thin fibrous structures intertwining among themselves which are painted pikrofuksiny in yellow color grouped in the form of the bunches going randomly or forming rhythmical structures.

Between fibers the oval or extended cells forming palisadoobrazny structures are located. In separate parts of a tumor find the little bodies of Verokai representing the sites deprived of kernels around which cells are located a paling that patognomonichno for Antoni's type of A. At Antoni's type of B prevails a parallel arrangement of tumoral elements in the form of the fine-fibered bunches surrounded with connecting fabric with existence in it of a large number of fabric basophiles. Mucoid transformation of a stroma of a tumor, sometimes with formation of small pseudocysts is noted, it is frequent with bloody contents. Dystrophic changes of neurolemmocytes are followed by massive vacuoles in cytoplasm, sometimes with myelin structures. In these cells, as a rule, there is no basal membrane. Between these types of a neurilemoma there can be intermediate forms.

Differential diagnosis

Neurofibroma, neuroma, leiomyoma.

General principles of treatment

Surgical excision.
