TROPICAL SPRU

This syndrome of an unknown etiology (perhaps, polietiologichny) is widespread in certain tropical areas; it meets in Caribbean countries, except for Jamaica, and in Africa, except the southern part of this continent. The syndrome is characterized by generalized malabsorption and diffusion damage of a mucous membrane of a small bowel.
Clinical manifestations. During epidemics people of all age, but first of all adults are ill. Temperature increase and an indisposition precede emergence of watery diarrhea. In several days acute symptoms abate, they are succeeded chronic malabsorption, by the alternating diarrhea and the anorexia which are often leading to heavy insufficiency of food. The last is shown by a night blindness, a glossitis, stomatitis, a cheilosis, hypostases, a xanthopathy and mucous membranes. Are noted the expressed muscular weakness and increase in a stomach.

Datas of laboratory indicate disturbance of absorption of fats, sugars and B12 vitamin. In bioptata of a mucous membrane of a small bowel the shortening of fibers and deepening of crypts expressed in various degree, a kruglokletochny infiltration of own plate, easy shortening of enterocytes are found. These pathological changes are not specific and benign can be observed at the healthy faces living in the same district.
Treatment. Appoint anti-diarrheal drugs and missing nutrients. Administration of folic acid and B12 vitamin in combination with oral administration of antibiotics of a broad spectrum of activity provides fast improvement of functions of a small bowel. Efficiency of antibiotics indicates participation of intestinal microflora in a pathogeny of this syndrome.
